How do I make an assignment to Quadmatrix Report™?
1. You may send an assignment via direct email and include all pertinent information for the type of assignment you are requesting. Just click on "ORDER REPORT" which will provide you with the appropriate email address for submitting your assignments; OR

What documentation will I need to include along when I submit my assignment?
For Medical Records Reviews and Peer Reviews, the client must forward to us in a .pdf format attachment, a copy of all medical records that have been provided to them related to the claim of interest. The client must clearly communicate to us, their desired turnaround time requirement for the assignment. We will send out a confirmation email acknowledging the receipt of the assignment, and provide the client with opportunities to provide any other relevant input they desire during this communication venue.
We do not require any additional documentation for investigation assignments as long as the assignment does not require us retrieving any consumer credit information or medical records from medical providers.
If your assignment involves us being tasked with the retrieval of consumer credit information, or medical records from medical providers, then you will need to provide a scanned copy of any permission or release document(s) signed by the claimant. You may provide this sending the release as an attachment via email to ClaimAssign@peer.report.
How long does it take for me to get my Quadmatrix Report™?
A complete Quadmatrix Report™ Investigation package usually has a 7-14 business day turnaround. If you order a single component such as a Background check, the turnaround time will usually be 2-5 business days. The report will be provided to you as an email attachment. Medical Records Review assignments turnaround time ranges from 1-10 business days and determined by the client.

What is contained in the Medical portion of the Quadmatrix Report™?
The claimant’s medical history and medical records are reviewed by our team of American Board Certified Medical Professionals to create an easy-to-follow analytical summary of all of the provided medical-related data. We utilize proprietary methods which have been developed by our team of industry professionals over decades. These methods provide our clients with a proven, powerful, relevant, medical knowledge-based tool that has historically succeeded in the claims industry repeatedly. Claim examiners and adjusters are able to efficiently conclude the settlement or disposition of their claims census with complete confidence by utilizing our proprietary services.
What is included in the financial portion of the Quadmatrix Report™?
This investigation may include searching for any current and previously owned assets, business involvements and affiliations, judgements, liens or bankruptcies. We also offer the option of completing a review of accounting records.
What are the components of the activity investigation portion?
Surveillance, Activity Checks and Social Media. You may order a multiple day surveillance to determine the extent of the subject’s physical social activities. We also have an option available referred to as an Activity Check or AC. An AC is a brief investigation, usually only taking 1 day to complete. This AC is formulated by utilizing proprietary methods of gathering personal information which provides a limited snapshot report of the claimant’s lifestyle. Social Media is another tool which may provide additional insight to the subject’s regular virtual activities.
What are the options when ordering a background investigation?
Basic options are: Civil, Criminal, Incarceration, Formal Education, Marriage, Divorce, Motor Vehicle Report, Real Estate Records, Birth and Death records.
